Who We Serve
We serve individuals 18 years of age and older with client groups such as: Alzheimer's, dementia, developmental disabilities, mental illness, advanced age, and more.
What We Provide
We help our residents not only with their daily routines, but to thrive physically, mentally, and socially within our community. We design care plans that meet the individual needs for each resident. We also have trained medical staffing available when needed. Our community offers a full activities and social calendar that helps residents thrive not just in body and mind, but also socially.
Facility Details
We have nine homes located in Sparta, Wisconsin. We offer residents the latest amenities in comfort that they would enjoy in the comfort of their own homes. Every home has private bedrooms, two shared bathrooms, as well as spacious closets that are great for storage. All residents have access to landscaped grounds and common areas.
Pricing
We do our best to make sure each resident is able to sustain a healthy, fulfilling life during their stay. To achieve this, we design care plans that meet the individual needs for each resident. Therefore, pricing is based on assessment. We accept private pay or Member Center Organizations.
